* Must reserve a spot 3 days prior to the group.

• Reservations can only be made by phone: 303.449.1951
• Let Children's Alley know that you will be attending Queer and Trans Parents Support Group (so they waive the fee).
• Spots are first come, first serve; eligible only to those with completed paperwork and who have made a reservation 3 days in advance. 

* Must complete all paperwork: 

• Enrollment Form Paperwork  
• Immunization record or exemption letter
• General Health Appraisal Form (signed by Primary Care Provider within 30 days of enrollment). 
• Child and Adult Care Food Program form
• Handbook Acknowledgement
• Signature on the income verification form
